The questionnaire will ask you to review an applicant’s set of application materials for a scholarship, and then
answer questions about the applicant based on those materials. Some of the questions ask about attitudes that
some people find unpleasant. There are two demographic questions that some people may find to be personal.
To address these risks, your responses are completely anonymous. No identifying information links your name
to your questionnaire and you may choose not to answer any question that makes you uncomfortable. We will
maintain anonymity by not collecting any names, email addresses, or other personally identifiable information
of participants.

This study will present you with a resume, personal statement, and photo of an applicant for a highly
competitive, LGBT+ exclusive scholarship. We will ask you to review these materials and answer some
questions about the applicant and how well they qualify for the scholarship. Additionally, you will be asked two
demographic questions about your own gender identity and sexual orientation.

These procedures will be conducted online using the Formr survey framework. The survey will be hosted on
Cloud Research and participants will be paid 45 cents for taking the survey.

This survey will take the following amount of time: 15 minutes (maximum)

Benefits: There will be no direct benefits to participants. Indirect benefits include contributing to the
understanding of the relationship between stereotypes of certain sexual minority groups and the effects of those
stereotypes on mental health and resource access.
Consent forms do not include identification numbers and data do not include participants’ names. You may
discontinue participation at any time without penalty or loss of benefits. Participation in the study is entirely
voluntary and you may decline to enter this study. Raw data and processed data will be made publicly available
through my Open Science Framework account at osf.io/yd6ta.
